We provide a wide range of aged care services to older people living in their own homes in the community, and also in our independent living units and residential aged care facilities. Aged Care Services include allied health & therapy services, home and community care, independent living units and residential facilities.

The Position:

We are seeking a dedicated and passionate individual who will be responsible for identifying and supporting older people to remain in the community through accessing appropriate advocacy supports and linking them where appropriate, to community care and other support services within defined eligibility criteria. Ensure older people, their families and carers receive an excellent customer service as you provide information and support about aged care, assist them to navigate the aged care system and support them with appropriate advice around housing and other support service needs.

Skills & Experience:

  • Understanding and commitment to social justice with knowledge of community services for older people and of duty of care responsibilities pertaining to community service provision.
  • Strong organisational skills in coordinating and administrating service implementation and delivery.
  • Knowledge of housing and homelessness issues involved in supporting and caring for older people in the community.
  • Able to be flexible and solution focused with the capacity to value diversity and difference using a range of approaches.
  • Excellent communication skills with sensitivity towards the needs of people from a diverse range of experience and cultural backgrounds.
  • Ability to network and collaborate with a diverse range of external service providers including advocacy groups, social housing providers, government agencies and community service providers.
